Medical certificate

Dormitory applicants should take a medical check up with their doctor. You will be requested to submit a medical certificate to the dormitory office when you check in the dormitory. There is no specific form for the medical certificate. However, it has to prove that you don't have any contagious diseases. (You may refer the sample medical certificate attached)
